Summary:
The Population and Community Health Program Manager is responsible for the daily management, oversight, and performance of their assigned projects in support of the Pediatric Group's team-based approach and commitment to patients, providers, and co-workers to provide optimal care.
The Population and Community Health Program Manager shall assess needs, develop, and implement policy & procedures around those needs, build the capability of the staff to meet those needs through training and perform the day-to-day functions that ensure the best performance of programs.
As a key contributor to maximizing healthcare delivery, the Population and Community Health Program Manager ensures each member performs their individual duties and contributes as a central part of the team by working together to provide coordinated and comprehensive continuity of care to our patients.
ESSENTIAL DUTIES & RESPONSIBILITIES (nothing in this job description restricts management's right to assign or reassign duties and responsibilities to this job at any time):
Manages the day-to-day coordination, administration, community engagement, and data functions for community health initiatives.
Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards.
Complies with governmental and payor regulations Acts as primary agent for the coordination and implementation of Population and Community Health initiatives including developing policies, procedures, goals, and objectives.
Plans and manages logistics for community events Assesses needs; analyzes and coordinates health teaching and education programs for target populations.
Assists in the analysis and presentation of data in clear, relevant, and visually appealing materials that can be used to make strategic decisions.
Creates and manages reports on community health, payor incentive programs and the like Coordinates and manages patient referrals and follow-up with specialists Participates in meetings, and on committees Participates in multi-disciplinary quality and service improvement teams Serves as a resource to colleagues and consumers of healthcare Ensure that staff is properly trained, and performance is continually assessed Collaborate with top management, interdisciplinary teams and other stake holds in order to develop, implement, and evaluate programs and services Review patient and staff data to measure the effectiveness of patient care and the quality of care Manage coordination and follow up of care managed patients Maintain and monitor compliance in meeting standards of patient care quality measures Promote a safe working environment in accordance with applicable laws and regulations including HIPAA, OSHA, CLIA and MOSH Adhere to and enforce infection control practices and policies Interpret and support corporate goals Participate in internal and external audits (ie state or insurance audit) QUALIFICATIONS - (Education and/or Experience):
The requirements listed below are representative of the knowledge, skill and or ability required:
BSN required from accredited program Minimum 3 years work experience including project management, population health coordination and staff development or continuing education Active RN license in the State of Maryland CPR certification required Communication skills:
excellent interpersonal skills, ability to read, analyze and interpret documents; ability to respond effectively to all inquiries; ability to write using proper grammar, punctuation, and spelling; ability to present information Strong analytical and critical thinking skills Strong community engagement and facilitation skills Strong attention to detail with the ability to multi-task and work within a fast-paced environment Computer Skills:
Proficient in Windows applications:
ability to use Excel preferred; word processing; spreadsheets, e-mail and Internet Proficient with electronic medical record Ability to deal with confidential information appropriately Relies on judgment and experience to plan and accomplish goals Ability to define problems, collect data, establish facts and draw valid conclusions Ability to work independently on projects and as a member of a team Flexibility and the ability to work autonomously as well as take direction as needed Job Type:
